I seek and request, of my own free will, to receive spiritual guidance from Generations House of Worship.
I understand that Generations House of Worship only offers spiritual guidance. Those desiring legal, medical, or professional counseling are encouraged to seek such services from licensed professionals. The staff and volunteer members of Generations House of Worship are not licensed professional counselors and do not provide counseling services. The assistance offered is spiritual in nature and is solely the guidance of a member of the ministerial staff or volunteer team of Generations House of Worship.
I understand that spiritual guidance is provided at no cost by a licensed, ordained minister or a layperson designated by the staff for this purpose.
I understand that all records related to spiritual guidance are the exclusive property of Generations House of Worship and not of the attendee.
I understand that many spiritual guidance follow ups may be a one time only encounter followed with next steps accountable to individuals seeking spiritual guidance.
I understand that all communication involved in the provision of spiritual guidance is treated as confidential. However, certain exceptions may apply, including but not limited to:
- The spiritual advisor seeking input from other church staff members for guidance.
- When required under Pennsylvania law, including disclosures of physical or sexual abuse under applicable reporting statutes.
- Under lawful order or subpoena from any civil or criminal court with proper jurisdiction.
- In cases of threats of self harm, suicide or homicide, when the minister determines there is a serious threat to the life of the one seeking spiritual guidance or a third party.
I freely and voluntarily agree to release forever Generations House of Worship, its ministers, staff, directors, officers, board members, and other representatives from any and all liability, claims, losses, or damages. This Statement of Understanding acts as a release and is binding on my heirs, assigns, or successors-in-interest.
The provision of spiritual guidance may be terminated at any time by mutual consent or at the request of either party.
